Gentamicin Calculator in Adults

This Gentamicin Calculator can be used for all Gentamicin Prescribing except for Infective Endocarditis or Surgical Prophylaxis at the Royal Bournemouth Hospital site. See the Adult Antimicrobial Microguide on this page for details.

• Use the Dose Calculator for the initial dose. Continue with this dose for all subsequent doses unless the patient’s weight has changed in which case re-calculate.
• Use the Interval Calculator to determine the timing of subsequent doses after a gentamicin level has been measured.
• A gentamicin level should always be taken after the first dose. Levels must be taken 6 – 14 hours post dose.
• In stable renal function, if course length is > 3 days, levels can be taken twice weekly.
