ICE requesting important reminder on consultant and location detailsThere have been many LERN forms submitted regarding ICE results going back to the incorrect consultant because the location and consultant details were incorrect on the request made in ICE. This could mean that a result is missed. This is a serious risk and missed diagnoses will continue to occur while these details are not being checked by the requester. It is the requesters responsibility to make sure the information on a request is correct. You must check the consultant and the location fields and change if incorrect before submitting the request to ensure the results are returned to the correct consultant i.e. returned to the consultant who the patient is under the care of. |
