Preoperative assessment - MyPreOp

Patients who require surgery under local or general anaesthetic have always needed to complete a health questionnaire prior to the procedure, to help spot any complications that might arise beforehand and discuss any concerns the patient may have. Historically, these always meant a trip to hospital to meet with a member of the preoperative assessment team.

Now patients can complete these important checks securely online at a time that's convenient to them, meaning there's no need to attend hospital for this service. These checks can highlight any relevant medical history a patient may have, any conditions that may need to be addressed before surgery, the most appropriate type of anaesthetic and even the way the surgery is performed. Completing your questionnaire online reduces waiting times for patients and makes it easier for our hospitals to plan your care.

The MyPreOp service is suitable for most patients, but there are some who may not like the idea of completing the questionnaire electronically, are unable to, or for whom it's important to meet an expert clinician before surgery. For these patients, face-to-face appointments are still available, and everyone has access to skilled preoperative assessment nurses by phone should they need it. What is MyPreOp?

MyPreOp is a pre-operative assessment questionnaire for you to complete at home via any internet enabled device. This assessment is necessary for us to start planning your surgery. The facility is provided by a company called Ultramed.

How do I access MyPreOp?

Patients awaiting a preoperative assessment with MyPreOp will be sent a unique code, via text message or email, for you to start your questionnaire online. If you are unable to complete MyPreOp by yourself, a relative, carer or friend can help you.

What if I cannot access the internet?

If you are unable to get access to the internet, please contact the UHD preoperative assessment team on the telephone numbers below and we will arrange to help with completing your MyPreOp questionnaire.

When should I complete MyPreOp?

Please complete your MyPreOp as soon as possible. Do not wait until you have an operation date, as we often do not plan operation dates until your pre-assessment is complete. Failure to complete MyPreOp could delay your surgery. When completing your assessment you will need to provide information on your medications, allergies, immunisations and height and weight. Please have this information available to you.

How long will it take to complete MyPreOp?

The questionnaire will take 30-60 minutes to complete and will ask you questions about your lifestyle, health, and your operation. You do not need to complete the questionnaire in one go; your progress will be saved as you enter your details.

Is the information I provide via MyPreOp safe?

Anything you enter into the MyPreOp assessment tool is stored safely and securely in line with strict NHS guidelines on information governance.

What happens next?

Once you have completed your questionnaire, the UHD Pre-Assessment team will review the information you have provided in preparation for your procedure. Should you require any tests or appointments, the Pre-Assessment team will contact you directly to arrange these.
